Transfer of Care Referral Process for Adjusters and Nurse Case Managers

If you are a claims adjuster or a nurse case manager, and your claim is in need of a Primary Treating Physician, please follow the steps below.

1.  Provide a Transfer of Care Authorization (9785 or 1305 letter) on carrier letterhead with the following information:

• Dr. John Alchemy named as the Primary Treating Physician
• Accepted body parts for the claim listed
• Fax to (206) 338-3005 or email* to Rhianna@johnalchemymd.com

2.  Send us the medical file

• Fax to < 200 pages to (206) 338-3005 or email* to Rhianna@johnalchemymd.com
• If records cannot be electronically sent, a CD or paper chart can be mailed to:
  • 4655 Hoen Avenue, Suite #4, Santa Rosa, CA 95405  (external link to Google Maps)

3.  Register the patient into our HIPAA secure portal: www.workslacker.com
