Request Administrative Services
Existing patients may request the following services:
Medical records requests
DMV placard documentation
California (EDD) disability forms
Doctor-completed forms
Private disability forms
These requests can be submitted using the appropriate downloadable or fillable forms.
Administrative Services & Fees
Administrative services are separate from clinical care and may not be covered by workers' compensation medical benefits. These services may include form completion, records requests, and employer or agency documentation. Fees may apply. Any applicable fees and processing expectations will be communicated before a request is completed.
Processing Times
Processing times vary based on the type of request and the completeness of the information submitted. The office will contact you if additional information is required.
